RELIGIOUS EXEMPTION DOCUMENTS

We are providing these documents for individuals that wish to seek a religious exemption for a mandated vaccine. Unfortunately, we cannot guarantee these forms will be approved by your employer, but we do believe that we should stand up against these mandates and fight for our religious rights. Please feel free to use all or parts of the information in our forms. Some of the information used to create these forms came from Andrew Torba on his blog.

Religious Exemption with Medical

This form has an extra part about walking in wisdom, and provides some resources where an individual could believe it is unwise to take the vaccine. The extra resources do mention some medical facts and statistics. We have been advised that mentioning the medical information could be confusing or counter-productive, but we are providing it incase someone wants to use it or edit parts of it. There is now a version two available. The second version adds a reference to the IL law that gives every individual the freedom to abstain from medical services based on their conscience.
